What version of the Codex App are you using (From “About Codex” dialog)?

Version 26.721.31836

What subscription do you have?

Plus

What platform is your computer?

Darwin 25.5.0 arm64 arm

What issue are you seeing?

Codex asks for access. I say no. Some time later it asks again.

What steps can reproduce the bug?

Running Codex without having granted permission - eventually (at a seemingly random time) it asks for permission.

What is the expected behavior?

(a) It's not clear why it's asking for permission in the first place. I have not knowingly enabled a feature that should require it.

(b) After refusing permission, it should not ask again. If necessary, it should show some UI indicating what features will not be available unless permission is granted, and it should then disable those features, and give me instructions on how to manually re-enable them if I want to later.

I can reproduce this as well on macOS.

I investigated the issue further and found the following:

  • Bundle ID: com.openai.codex
  • Reinstalling the app does not resolve the issue.
  • Resetting TCC does not resolve the issue:
tccutil reset SystemPolicyAppData com.openai.codex

After allowing the permission again, TCC shows:

com.openai.codex|kTCCServiceSystemPolicyAppData|5|2|0|...

The permission dialog continues to appear repeatedly.

Additional observations:

  • ChatGPTHelper remains running in the background.
  • No apps are enabled under macOS "App Management".
  • "Computer Use" is not configured.
  • There are no OpenAI LaunchAgents under ~/Library/LaunchAgents.

This seems related to SystemPolicyAppData / App Data Access rather than any specific application integration.
