Hello OpenAI Support,

I am unable to use Codex at all — every attempt to create a task fails with the error:

Failed to create task

This happens no matter what I do:

Clicking on predefined templates (e.g. “Try in your terminal,” “Try in your IDE”).

Entering a custom prompt directly.

Using different browsers (Chrome, Edge) and incognito/private mode.

Logging out/in and clearing cache.

Triple-checking my environment setup.

The error has persisted for weeks; I don’t think Codex has ever worked for me.

Account plan: [insert yours — e.g. Plus, Pro, Business]

Browser/OS: [e.g. Chrome 139, Windows 10]

Repo: connected (shows in UI)

Error message: “Failed to create task” (see attached screenshot)

Reproducibility: 100% (every task, every template)

Impact:
Codex is completely unusable — no tasks can be created at all.

I’ve seen other users report the same problem on the OpenAI Community Forum (e.g. “Codex failing to create new tasks and follow-ups”), so it appears to be a service-side bug. Can you please confirm and advise next steps?
