Codex repeatedly replies with old historical questions and echoes my input content for new queries

What issue are you seeing?

When I send brand new unrelated questions to Codex, the model fails to respond to my new prompt.
Two abnormal behaviors occur consistently:

1. Codex automatically reverts back to discussing my previous old conversation question instead of handling the new request.
2. At the end of its reply, it directly echoes and outputs the full text content I just sent as my new query.

Reproduction Steps

1. Start a conversation and ask Codex an initial question, finish the first round of dialogue.
2. Send a completely new, unrelated question with new input content.
3. Observe the model output: it talks about the old question and repeats my new input text at the end of the response.

Expected Behavior

Codex should ignore historical unrelated context, answer the newly submitted question accurately, and not repeat my input prompt as output content.

Additional Info

No special custom prompts / plugins enabled. The bug occurs every time I switch to a new question.

What steps can reproduce the bug?

1. Start a conversation and send an initial question to Codex, complete one full round of dialogue.
2. Send a completely new, unrelated question with brand-new input content.
3. Observe the model output:

• Codex ignores the new question and responds to the old historical question instead.
• At the end of the reply, the full text of my newly submitted query is echoed and printed directly as output.
