Codex view remains blank on VS Code 1.128 (Windows 10)

What issue are you seeing?

The Codex extension installs successfully and activates correctly.

However, the Codex UI never loads.

Symptoms:

• Codex: New Codex Agent opens a blank page.
• Codex: New Chat in ChatGPT Sidebar is blank.
• Chat: Manage Language Models shows an empty list.
• No OpenAI login screen is shown.

The extension appears in Running Extensions and activates successfully.

Things already tried:

• Reinstalled the extension
• Switched between Release and Pre-release
• Developer: Reload Window
• View: Reset View Locations
• Fresh VS Code User Data
• Browser access to chatgpt.com works normally
• ChatGPT Plus account

Expected:

Opening Codex should display either the login flow or the Codex UI.

Actual:

The Codex page remains completely blank.

The issue reproduces in a fresh VS Code user-data directory with no custom settings.

What steps can reproduce the bug?

1. Install the Codex extension.
2. Open VS Code.
3. Run "Codex: New Codex Agent".
4. The Codex view opens but remains completely blank.
5. Run "Chat: Manage Language Models".
6. The Language Models list is empty.
