“Allow once” button is unresponsive in the file edit permission dialog

Whenever Codex displays the file edit permission dialog asking, “Allow ChatGPT to edit the following files?”, clicking the “Allow once” button has no effect.
The dialog remains open, the proposed file edits are not applied, and Codex cannot continue the task. There is no visible error message or other response after clicking the button.
The only way to leave this state is to close and restart the Codex App. However, restarting does not resolve the underlying issue. As soon as another file edit permission dialog appears, clicking “Allow once” causes the same problem again.
This makes it impossible to use Codex for tasks that require editing files.
What steps can reproduce the bug?
- Open the Codex App.
- Open a local or remote project.
- Ask Codex to perform a task that requires modifying one or more files.
- Wait for the dialog asking, “Allow ChatGPT to edit the following files?” to appear.
- Click the “Allow once” button.
- Observe that nothing happens: the dialog remains open, the files are not modified, and Codex does not continue.
- Close and restart the Codex App.
- Trigger another task that requires file editing.
- Click “Allow once” again.
- Observe that the same issue occurs again.
The issue is consistently reproducible.

I'm seeing the same failure on Codex Desktop for macOS with a remote Linux workspace.
On 2026-07-31, this stranded three separate tasks. In each case:
active/waitingOnApproval;fileChangeitem remainedinProgress; andTwo affected tasks originated before a core upgrade. I then reproduced the same deadlock in a newly created task running Codex core
0.146.0-alpha.9.2, so it does not appear limited to stale pre-upgrade tasks.The same edits completed successfully in a separate task without interactive approvals, suggesting the failure is in the approval lifecycle rather than the patch contents.

I encountered this issue again today while using Codex to operate a remote server. This time, even after quitting Codex and restarting it, Codex still could not run properly and remained stuck. Restarting the application did not resolve the problem.
I am experiencing the same behaviour working on a remote project. Codex updated to ChatGPT app yesterday: ChatGPT 26.727.51351 on Mac. The "Deny" button also doesn't work incidentally.
