Codex project chats become unusable with repeated “Request failed with status 404” after Word file upload error

I am working inside a ChatGPT/Codex project. The problem started when I attempted to upload a Microsoft Word document that was still open in its project folder. The upload failed.
After that error, the project chat became unusable. Almost any new request now returns:
Request failed with status 404
I created a second chat inside the same project to continue working, but that chat eventually developed the same problem and could no longer be recovered or used normally.
I am now on my third chat inside the same project, and the same 404 error is occurring again in response to ordinary requests.
This does not appear to be related to a specific prompt. Once the problem begins, even simple messages can trigger the 404 response. The affected chats contain important project history and context, so creating a new chat each time is not a sustainable workaround.
At least two previous chats in this project have already become inaccessible or unusable.
What steps can reproduce the bug?

What is the expected behavior?
A failed file upload should produce a recoverable upload error without corrupting or invalidating the current project chat.
After the upload error, I should still be able to:
Continue sending ordinary messages in the affected chat.
Close and reopen the chat without losing access.
Create new chats within the same project without inheriting the failure.
Access and recover the conversation history from the previous affected chats.
The application should not return a generic HTTP 404 error for normal prompts, and one failed upload should not cause multiple chats within the same project to become unusable.

This is occurring in the Codex desktop app on macOS with Apple Silicon.
My subscription is ChatGPT Plus.
The problem appears to be scoped to this particular project.
The original trigger was a failed upload of an open Word document.
This is now the third chat I have created in the project because the previous chats stopped functioning.
Two previous chats contain important work that I have been unable to recover.
Restarting by creating a new chat only works temporarily and does not resolve the underlying issue.
The error shown to the user contains no additional diagnostic information beyond:
Request failed with status 404
Because the failure affects multiple chats and project history, this may involve a corrupted project reference, conversation identifier, attachment reference, or project-level state after the failed upload.
