OpenAI API key setup: local destination confirmation returns declined without showing a prompt

Summary

When creating an OpenAI project API key through Codex desktop, the OpenAI Platform target picker works, but the local destination confirmation does not appear. The connector immediately returns:

The local API key destination was not approved. Do not create or write a key.

Steps to reproduce

1. In Codex desktop, ask to create a project OpenAI API key.
2. Select a key name and organization/project in the OpenAI Platform widget.
3. Continue to the local destination confirmation for .env.local.

Expected behavior

A confirmation dialog appears so the user can approve or change the local environment-file destination.

Actual behavior

No confirmation dialog appears. The tool immediately reports that the destination was not approved, so the key cannot be created or saved.

Additional context

• Reproduced repeatedly in the same session.
• Reproduced after restarting Codex and restarting the setup flow.
• The key was not created or written.
• No API key, account secret, or sensitive value is included in this report.
