Not showing changes to make so a user can accept or refuse them

Which model were you using?

gpt-5 gpt-5-codex

What platform is your computer?

Ubuntu 24.04, x86

What steps can reproduce the bug?

Just run the cli, tell it to make changes.

What is the expected behavior?

Usually, when i choose "2. No, ask me to approve edits and commands"

, it shows changes to be made i accept them or refuse them. but i doesnt do that anymore for some reason.
I will never allow it to run commands without my consent.
