VSCode Codex 0.4.10 is not following orders well, it provides less explanations and doesn't show the files changed

What steps can reproduce the bug?

I asked Codex to add a small feature to a 400 lines file, what I got in exchange is very little explanation, I didn't get the diff that I used to get, and somehow the file went from 400 lines to 800 lines

What is the expected behavior?

To get the diff in the files that were changed, to get better explanations of what it actually did and to follow orders better without adding a ton of garbage code, basically go back one version
