Summary
A generated .docx file was present on disk and valid, but the Codex desktop response rendered its file attachment/citation as an empty bullet. A Markdown file link in the next bullet rendered correctly.
Steps to reproduce
- Ask Codex to create a DOCX document.
- Have the final response include the generated DOCX as a local file output/citation inside a Markdown list.
- Open the completed response in the Codex desktop app.
Actual result
The DOCX item appears as a blank bullet with no filename or clickable attachment. The following Markdown link renders normally.
Expected result
The DOCX filename should be visible and clickable/downloadable.
Additional details
- The DOCX existed at the cited absolute path.
- The file passed ZIP integrity validation and rendered correctly to three pages.
- Reproduced on macOS 26.6 (25G72) on 2026-08-10.
- The affected task ran for 10m 42s, and the user then had to spend another turn requesting the file again.
- Screenshot is available from the reporter if needed.
