Codex ran shutdown -h now as per instructions when I've gone to bed at night when I turn on the laptop again in the morning I can see the model being interrupted as the process is killed due shutdown.
then I picked up the thread again via codex resume and continued for some time but when I closed it and gone to revisit again the issue appears that the history is from the yesterday night model interruption point after it executed to shell tool shutdown -h now
now the thread can't be resumed but the work is logged
What steps can reproduce the bug?
- keep codex running inside the CLI
- ask the model to run
shutdown -h now - visit the thread back via codex resume and send few more messages
- close the re opened thread and try to open it again (the new history is gone with only access point for that being the logs)
What is the expected behavior?
the updated history should populate in the same thread ideally if any issues with that then alteast in a new forked thread

This looks like a good case for an independent rollout-level check. Codex Rescue doesn’t rely on the thread-store projection to inspect the local session, so it may still be able to diagnose the persisted history and create a safe handoff even if

The "ordinal mismatch" in your logs places this in a known family: the thread-history projection cursor (SQLite) has fallen out of sync with the rollout file — here almost certainly because
shutdown -h nowkilled Codex mid-write, leaving a torn cursor. The projector on currentmaindetects the mismatch on every subsequent attempt and refuses to advance (hardInternalerror), but has no repair path — so the projection freezes at the pre-shutdown point forever while the rollout keeps logging, exactly what you observe. Full mechanism, with the invariant-check code and a proposed auto-reprojection fix: https://github.com/openai/codex/issues/38792#issuecomment-5322845850Workaround verified in that thread: with Codex fully stopped, delete the affected thread's rows from
thread_items/thread_turns/thread_history_projection_statein~/.codex/thread_history_1.sqlite— the projection rebuilds from the intact rollout on next resume, restoring the lost turns. (Back up the file first.)
