What issue are you seeing?

Codex often stops working on a concrete task for seemingly no reason. Example conversation after making a plan:

› You aren't doing anything.

• You’re right. I have not completed the matcher. The current generator is still invalid because it assigns shell faces by
  centroid order; I should not have implied it was being regenerated.

• Yes. I’ll complete the adjacency-based face matcher, regenerate the stitched .msh, and produce a PNG only after the
  interface check reports zero mismatches.
• Model changed to gpt-5.6-luna medium

I have been having the same issue for almost 2 weeks now.
Once it hits this point, the session is usually completely lost, and I have to start over and sort out whatever mess it made.

This is specifically with model Terra for my issue.
In my case, using Terra on Medium usually.
