Environment setup hanging indefinitely

What steps can reproduce the bug?

I have an open session on the web version of codex that has been stuck at "Setting up environment" for an entire week now.

I got into this state by attempting to stop the task exactly as it was starting. Clearly this caused some bad state which I'm still stuck in. Link to my session:

What is the expected behavior?

I'd have expected the environment setup to abort and the agent would return to neutral state.

I am not sure if this is the same issue, but I am getting the same message when either:

Attempting to start 2 tasks in codex one after the other - generally in unrelated parts of code, or one is a code and one is an ask
Asking a follow up code question in an existing code modification, ie, fine tuning an MR

I'm still having the same problem. Any updates on this? I tried deleting the environment and creating a new one, but the issue persists.

We have investigated the task launch process and made some changes that should theoretically catch these going forward. Please re-open this if you get new borked tasks and we'll investigate!

I just had this. Created a PR and OpenAI to automatically review but when it spawned the instance in web, it's just hanging here

I am having this issue right now as well. Would love to know if any fixes are found.

I'm also having this issue. Seems to hang on env setup, if cancelling and re-mentioning codex - it will comment on your PR with

Codex Review: Something went wrong. Try again later by commenting “https://github.com/codex review”.

Unknown error

I then tried to remention codex with "review" - but the same ocurred, hanging on env indefinitely. @ddr-oai

I found a workaround that works for me (I am using ChatGPT codex). If you up the simultaneous versions to 2...it works every time for some reason BUT it takes away from your usage. Sounds kind of like a money grab to me because it ALWAYS works when i use more than 1 to do it but i never get credited when it takes up my credits with this garbage second version which i dont want or need.

I also have this issue with code reviews requested via GitHub. The resulting code review execution that I can see in Codex Cloud hangs with an empty "Environment setup" as in the screenshot above
