This is a regression from 0.147.0 and before.
I run codex with app-server CLD and a -c option of mcp_server.myname.url="http://localhost:3001/mcp"
This has worked for a very long time. As of 0.148.0 requests to use the tools fail with an error message that says that the tools must be approved by the user.
What steps can reproduce the bug?

mcp tools explicitly added via the command line should be auto approved as they have always been.
