Which model were you using?

GPT 5 Codex

What platform is your computer?

Microsoft Windows NT 10.0.26100.0 x64

What steps can reproduce the bug?

Codex appears to be unable to correctly read a file after making a manual edit and saving.

Make a manual edit in any file that was generated by codex and ctrl+s to save the file. Ask codex via chat (Vs Code extension) to then review that file for the new change, it does not spot the change. Refer to screenshots below.

What is the expected behavior?

That codex is able to correctly read a file when I ask it to in the chat. Auto context is selected.
