Which IDE are you using?

What platform is your computer?

What steps can reproduce the bug?
this continues to be a bug and was not fixed.

What is the expected behavior?
Approvals should never be asked for when full access is requested.
What do you see instead?
Constantly requires individual approvals for every little action -- every powershell command.
Additional information
Tried all kinds of wild cards and settings in config.toml to stop these approval requests. This problem makes the extension almost completely unusable.

Same for me with 0.5.12
Same problem on my side
A quick solution is to run VS Code as administrator
Sadly, no changes for me
Moving to WSL fixed things for me. Recall that codex CLI support on windows is still rudimentary.
