[windows] codex IDE extension - full access continues to request manual approvals

What steps can reproduce the bug?

this continues to be a bug and was not fixed.

What is the expected behavior?

Approvals should never be asked for when full access is requested.

What do you see instead?

Constantly requires individual approvals for every little action -- every powershell command.

Additional information

Tried all kinds of wild cards and settings in config.toml to stop these approval requests. This problem makes the extension almost completely unusable.
