Model Terra refuses to complete tasks

What issue are you seeing?

Terra model is constantly acting like a rebel 13yr old for the last 1.5-2 weeks now
Mid-session, the model will suddenly refuse to complete a basic task or prompt, even when it does not violate any sort of safety or security requirements

Tasks like "please split this pr in to multiple pr's" it will complete the first pr.. then just stop and all follow-up prompts just refuse to do anything at all, admitting it didn't complete the task while also not actually doing the task when requested to continue.

I have had this happen across dozens of sessions.

What steps can reproduce the bug?
