Codex CLI conversation was interrupted during a read-only recovery/recon workflow after an approved PowerShell command. The session reported: “Conversation interrupted - tell the model what to do differently. Something went wrong?” Feedback upload succeeded.

Observed behavior:
- An approved command completed and returned a large diff.
- A subsequent request was canceled/interrupted.
- A following read-only command returned no output.
- The conversation then terminated with the interruption message.
