What issue are you seeing?

Codex fails to resume an existing CLI session after the previous process is interrupted while waiting for command approval and Codex is then restarted with --approve-for-me.

The same session can be resumed successfully when Codex is restarted normally, without --approve-for-me.

The error is:

Failed to resume session from ~/.codex/sessions/.../<session>.jsonl:
thread/resume failed during TUI bootstrap:
thread/resume failed: thread <thread-id> already has an active writer
(code -32600)

This suggests that interrupting a pending approval may leave the thread with a stale active-writer state, or that changing the approval reviewer during resume causes the new process to incorrectly detect an active writer.

What steps can reproduce the bug?

Start Codex normally:
1.codex
2.Ask Codex to run a command that requires user approval.
3.While the approval prompt is displayed, interrupt Codex using Ctrl+C.
4.Restart Codex with automatic approval enabled:
codex --approve-for-me
5.Resume the interrupted session using the session picker or:
codex resume
6.Resume fails with:
thread/resume failed during TUI bootstrap:
thread/resume failed: thread already has an active writer
(code -32600)
7.Exit and restart Codex without --approve-for-me:
codex resume
8.The same session resumes successfully.
