Codex weekly limits hit on first prompt in VS Code extension

What steps can reproduce the bug?

I use the CLI a lot in a windows power shell under WLS2. I am on the OpenAI plus plan. I have now attempted to use the codex vscode extension 3 times exactly and only one prompt at each attempt. Each time it runs a few seconds and then alerts me I have hit the weekly limits on the first prompt. The first two times, I just thought were a coincidence, but I have never once hit the weekly limit using the CLI in WSL. The third time I had barely used codex over the weekend and tried VS Code extension first thing on Monday and I immediately hit the weekly limits on the first prompt. I am certain that this extension is inflating the usage or causing my account to have incorrect timing or something. The CLI in WSL is then locked out as well.
