What steps can reproduce the bug?
It's an issue for codex web, not codex itself.
I don't know where to report, thus hereby leaving this issue.
When creating an environment, forked github repo does not appear.
I can choose repos I created, but not for the forked ones.

I believe this is a bug.

I'm also seeing this issue; some other information from what I've seen personally:
As a workaround, you can restrict Codex's authorized repositories in GitHub to just the necessary repository(s), and then select it from the general list. With the environment(s) created you can switch back to whatever setting you had before.
