Editing File in read mode (with request approval) does not Work

What version of Codex is running?

codex-cli 0.44.0

Which model were you using?

gpt-codex-high

What platform is your computer?

Darwin 24.6.0 arm64 arm

What steps can reproduce the bug?

  1. Toggle /approval and set to read with request for approval
  2. Ask codex to implement change, it fails to edit
  3. Mention clearly: "YOU CAN ASK FOR EDIT APPROVAL REQUEST so this environemnet is not just readonly !!"
  4. It keeps failing

What is the expected behavior?

Codex should ask for edit approval and actually edit the file.

What do you see instead?

Codex doesn't edit the file.
