What steps can reproduce the bug?
- Toggle /approval and set to read with request for approval
- Ask codex to implement change, it fails to edit
- Mention clearly: "YOU CAN ASK FOR EDIT APPROVAL REQUEST so this environemnet is not just readonly !!"
- It keeps failing

I looked into this and found that the "Don't Trust" option wasn't applying the approval policy correctly. Opened a PR to fix it: #4941
