Bug: codex (web) ignores information from images

I have an ongoing project and I uploaded an image containing an image that includes a mockup with specific instructions on it.

I tried this repeatedly...but instead of doing the task based on the image as told, codex ignores the image completly and just randomly makes up stuff for itself to do / implement.
