What steps can reproduce the bug?

This is for the Codex Extention (in VS Code Insiders- Local, Agent (full access), gpt-5-codex high). NOT CLI!

When I drag in an image to reference, it doesn't populate or give any visual indication that it acknowledges it- however, after click the '+' and adding the image from my file explorer, it then will recognize it twice.
