Can you provide more details about how you've configured codex? What are the relevant lines in your
~/.codex/config.tomlfile?config.toml (1 line only):

Thanks for the update. I don't have a working theory on this yet. I'm not able to repro the problem, and to my knowledge no one else has reported this issue, so it must be due to some set of factors that are specific to your configuration.
