What issue are you seeing?
During the task work, I am getting this error more frequently lately. I tried to click return to the message list and go back to the session which errored and still see it. The only way to go back is to close and reopen VS Code.
I don't know where to get logs from, so it may help you to debug.

I've been getting the same issue for a while now. A few things I have noticed, it only happens when I work on frontend (I'm on an Angular 19 project), and it also only happens when its building the final summary.
The bigger the chat, more change of it happening, and if it happens once, it will happen almost always in that chat.

im experiencing the exact same issue
in Cursor, working on Nextjs project, from time to time i get this error, try again literally do nothing, so i have no idea what for to put this button there if it do not do anything, even when you go on the top, and try to call Codex again, the error will appear, I mentioned that this may be caused due to memory load, only reloading completely is fixing the issue, but the reasong why it appear do not clear for me.
yesterday i got it 3-4 times, today i woke up, i went to chat, i didnt even sent request, only started typing my request prompt in chat, and then got this error again, this time no memory issues, and have no idea why it happen.
