• Version: Codex CLI 0.53-ish (latest).
• Expectation: registered notify hook should receive approval-requested.
• Actual: approval prompts appear in the UI but only agent-turn-complete

events reach the hook; manual invocation works.

• Evidence: ~/.codex/log/codex-apple-voice-notify.log missing entries, plus

manual test output.

approval-requested is never logged.
