Codex keeps forgetting to use Serena MCP after a while.

Which model were you using?

gpt-5-codex (reasoning high, summaries auto)

What platform is your computer?

What issue are you seeing?

Codex, after some time or in the middle of a long run, will forget to use Serena MCP for file functions. It's good about using Serena for searching for strings, but doing reads and especially writes, it'll fall back to direct manipulation. Interrupting it, and reminding it to use Serena will put it back on track.

What steps can reproduce the bug?

What is the expected behavior?

Not forgetting to use Serena.
