What steps can reproduce the bug?
Put custom instructions markdown in ~/.codex/instructions.md.
What is the expected behavior?
Custom instructions appended in a new section of the main prompt markdown.
What do you see instead?
Custom instructions markdown becomes part of the last list item of the main prompt:

This was fixed in https://github.com/openai/codex/pull/578, will be part of next release

I'm now running from sources (master) but still seeing the issue, would you please check?
