gpt-5 (reasoning none, summaries auto)

What issue are you seeing?

It is wasting my time, not being able to read a complete design document and create a UI design. I capture screenshots and annotate them, but it cannot act on those and returns the same old garbage over and over again.

What steps can reproduce the bug?
