What issue are you seeing?

Codex finished certain task and stopped for review as instructed before. But when told that review done, didn't follow up as instrcuted in AGNETS.md. when I wrote the instructions into promt - same result, didn't do anything, same ahhpened for 3 more times until i decided to provide feedback
