The model often adds fallbacks. Almost all fallbacks are hacks or workaround for various issues.
What steps can reproduce the bug?
Look for "fallback" in comments. Pretty much every time you see the word "fallback" in the comments, the model made a workaround for a bug or other issue, temporarily hiding the real issue.
What is the expected behavior?
The model should try to fix issues instead of adding fallbacks.
Additional information
Adding a no-fallbacks policy to AGENTS.md helps alleviate the issue.
