Codex keeps asking for permissions after pressing 'Yes, and don't ask again for this command'
What version of Codex is running?
codex-cli 0.56.0
What subscription do you have?
Plus
Which model were you using?
gpt-5-codex
What platform is your computer?
Ubuntu 20.04 - Linux 5.4.0-215-generic x86_64 x86_64
What issue are you seeing?
While inspecting files and folders it will run commands like
rg .....
rg ... | sed -n
And generally commands that will limit the output to a few lines
When it asks for permissions for those for lines 10-24 for example,
I select option 2. Yes, and don't ask again for this command
However as soon as the line number changes to for example 50-65 it will repeatedly ask for permissions for just switching the params.
Moreover these are read-only commands so they have no detriment, they should have been proceeded without permission requiremtns at all.
What steps can reproduce the bug?
It usually happens to me if I have a problematic linux service that wont' start and I ask the model to investigate the root cause. It has to go through many system files and journalctl and do paging to gather data from logs.
What is the expected behavior?
After I press 2 it should identify the nature of the commands e.g. is it only chunking a file's contents? is it read only ? And respect the decision and not repeatedly ask for new permissions

Similar to #5491 and #5199 so closing
Clsoed 3 months ago but the issue clearly persists... Today I had to approve permission for running the same command with different variation 3 dozen times in the same answer... but yes, just close the issue, who cares for the paying customer's time anyway?
Same issue here (on Windows), this is an issue that persists since several months and it's extremely annoying.
Still happening lol
It's happening to me too. All the time asking for permission, repyling with grant access and seconds after asking again to edit the same file
Been experiencing the same issue.
I'm experiencing the same issue right now
Makes the product nearly unusable tbh. Moving back to Claude. Wish Openai gave a shit.
Happening for me, super annoying when making large changes.

yup its annoying.
Still happening. Any fix?
still the same...
cannot go to sleep, otherwise codex will get stuck
sama sama
So while it's not the best, if you give a solid prompt instruction, I marked permissions as Full Permission. It fixed that issue. Also you can ask your Codex Agent not to use shell commands and to edit files directly instead of going through shell. That also helps.
Still a problem. It's a bit annoying having to sit watch it work.
Still a defect. This is the best argument against using Codex I know of. The people at Cursor appreciate you.
Still a problem
Still a problem for me as well
Still a problem +1
Same problem. I lose so much time because of this popping up a dozen times

Still an issue, "Yes and don't ask again.." just doesn't seem to work.
This make Codex barely usable now
This hasn't been fixed by OpenAI since more than half a year?
