Same issue here,

  • MacBook Pro M3,
  • running Sequoia v15.5
  • Codex Max subscription
  • tested on a variety of max/non-max and regular/mini models, with prompt "test"
  • tested on NPM package versions 0.63.0, 0.61.0, 0.64.0-alpha.2

Thanks for reporting the problem. The service is down in some geographies. The team is working on it. Closing in favor of https://github.com/openai/codex/issues/7156.
