[VSCode extension] GPT-5.1-Codex-Max loosing ability to open/read files

What issue are you seeing?

It happened multiple times already. At some point (more than 20% context size) the model responds that it has no access for tools, ability to read files (even with direct '@' context mark). I've seen in other conversation that in the CoT it start to test simple cli commands like ls, ripgrep, cat etc. without luck.

Sometime when explicit instructed to use avaible tools it regain the access for shor time. In general i'm forced to start new conversation.

What steps can reproduce the bug?

Hard to identify. This wan't the case before the GPT-5.1-Codex-Max introduction.
