Been happening for weeks now, happens with higher tier models, rarely ever happens with low reasoning...
Made a report about this previously:

Thanks for the bug report.

A few follow-on questions:

  1. Are you using any non-default configuration parameters? For example, are you using an LLM proxy?
  2. Are you using a VPN?
  3. A "Reconnecting" message is emitted when an error is received from the model endpoint that we think is non-fatal. We retry the request up to five times before failing. Are you seeing it get to the fifth failure, or does it appear to go through after one (or a few) retries?

Not using any VPN, proxy or any other weird network monitoring software
I am running it on my VMWare ubuntu 22.04 installation
Often it will actually attempt to continue, but eventually it usually fails and hits 5/5 and I have to start the whole request again

If I switch to a lower model like GPT-5.1-mini it will rarely have reconnecting issues

Same issues for 5.2-codex in v0.75.0. MacOS 15.6.1
5.1-codex-mini works just fine, 5.2 high too.

Add "DO NOT use functions.apply_patch" in AGENTS.md and use filesystem mcp from modelcontextprotocol. This may help.
