Having the same issue at ramdom moments. Need to ask to 'apply it' to make it actually apply the patches.

Sometimes I say "please apply it" or "nothing changed, please apply that changes", and it process again and still dont apply.

my team and I have also experienced this quite a bit. IME it happens occasionally with o4-mini and quite consistently with gpt-4.1. would be great to have this fixed as it is frustrating + makes automation with Codex tricky
