What steps can reproduce the bug?

Have codex make a patch
Open the diff viewer
Try to click on a line of the diff

What is the expected behavior?

Click on a line in the diff or the file title in the diff viewer
Opens the source file to the line that was clicked

Same! Regression in navigation:

Previously, clicking the filename in “X files changed” opened the file/diff.

Then it only worked after expanding and clicking a code line.

Now neither the filename nor the diff lines navigate anywhere.

Please fix “open file from diff” in the webview.

Windows VS Code ssh-remote to the Ubuntu 20.04 with Linux local installed Codex Extension

I'm finding something similar to be 100% reproducable - I have multiple files in my diff (in the codex mac app latest) and when i make a change to the first file, it updates automatically. but any updates to the second file, do not show. they show in my vs code ide, but not in codex app. even if i go to "refresh" it doesn't update. the only fix is to collapse and re-expand the file, then it refreshes. i have to do that every chagne and of course i lose the place i was at.
