What steps can reproduce the bug?

After a few turns of interaction, this error consistently occurs. Restarting Codex or instructing it to continue allows a few more steps to execute, but the same error reappears shortly afterward. When I downgrade to version 0.58.0, the issue no longer occurs; however, that version does not support codex-max, which prevents me from using the required model.
