Codex doesn't persist previous approval level

What subscription do you have?

Which model were you using?

What platform is your computer?

Darwin 24.6.0 arm64 arm

What issue are you seeing?

Codex selectively persists the selected approval level.

In some repos it persists it and it does not need to be set every single time. In other repos, it doesn't and the user needs to remember to set the desired approval level every single time, which can lead to the frustrating situation of starting a long operation, and then either having to give manual approvals for everything or stop, set the correct approval level, and then restart.

Uninstalling codex, and deleting the hidden folder does not fix the issue either.

What steps can reproduce the bug?

It's not consistent but when it happens is always reproducible.

  1. Set new approval level
  2. End session
  3. Approval level resets to read only.

What is the expected behavior?
