The extension has worked without issue up until earlier today when it just stopped working. I re-installed codex via brew install codex this morning.
The codex CLI tool is working without issues.
Reinstalling the VS Code extension did not help. The extension still works for Cursor run in remote ssh sessions on an Ubuntu server.
The lack of error logs, and instead just a Oops, an error has occurred message is not helpful. I can see why others could not provide many details: https://github.com/openai/codex/issues/5930

Dear community and Open AI stuff, we need your help, please do not close the issue until we collect enough information from the users to catch the bug that is really annoying. I understand, that your goal is to close issues as fast as you can, but we have not enough info to prepare complete bug report.
"Oops, an error has occurred" and "Try again" button on white background
What steps can reproduce the bug?
When using Codex Extension, regular development and tasks, as usual, some times small, some times bigger getting "Oops, an error has occurred" and "Try again" button on the white background.
It is not clear enough how to reproduce the bug, but it happen to me 3-4 times yesterday and 2 times already today,
What is the expected behavior?
"Try again" button reload the extension properly without reloading IDE completely

How to resolve? completely reload IDE only helps
Which is really annoying as far we are using different free and paid CLI and extensions, and some times you need to wait till accomplish the task within that particular CLI (aka Claude).
This is causing next thing - when you are waiting for Claude to reload, you work with Codex, then Codex crashed, and whole progress stops, because you can not reload IDE, as far you would lose the progress you made with Claude, and Codex is stack on this "Oops, an error has occurred" state.
If you could instruct me where to get logs, i would gladly provide them, if they even exist.

I didn't find any useful information inside "Toggle Developer Tools":

My assumption is that the issue may be caused by the output window containing too much text, which could be triggering an error. It might be helpful to archive older content to reduce the load, or simply delete excess data once it exceeds a reasonable threshold.
i been running into this issue for every prompt session across all projects and "chats" within 5-30min for the last 9hrs or so.
about 90% of the time i cant get the session to re-open when i get the error, i have lost alot of work due to this :(

Just upgraded Codex to 26.513.31313 on Windows 11, and I am not able to view any of my chats. I keep getting "Oops, an error has occurred". How can I downgrade to the older version temporarily?
