Visual Studio Codex Extension crashes a lot

What issue are you seeing?

Sometimes working the extension crashes. Sometimes produces blank panel in visual studio code. All work lost, no history is recorded. A chat cannot be resumed from the crash point. All information is lost form execution start. Let's say you start a task with some context. If the extension crashes in history there is only visible the previous context but not from prompt -> crash. This is probably a separate bug.

What steps can reproduce the bug?

I have not figured out how to reproduce it. Crashes sometimes randomly. Very unstable.

What is the expected behavior?

No crash. And if it crashes should be instrumented to report the crash log to a server so OAI engineers can track the issue. Example with relevant stack traces and application state.
