What issue are you seeing?

When using Codex in chat mode, whenever I ask if something can be done, it requests approval at the end for any code changes. This should only apply to agent and full access modes.

What steps can reproduce the bug?

Ask what steps need to be taken for an implementation using the chat mode.

What is the expected behavior?

Approval questions shouldn’t be required when changing code in chat mode.
