`Implement this Plan` question needs an _optional notes_ field

I love the new Plan collaboration mode. Love how it asks you questions and allows selecting a recommended reply along with an optional note.

please allow us to provide Codex with an optional / additional note. This could be a reminder, an addendum, a by the way sort of instruction. Since codex gives you this option only _after_ it shares the entire final plan with you, reading through there may be a "Oh by the way" note I'd like to add to make sure it keeps that in mind.

In my case, Codex frequently gets lost when trying to add new files to Xcode - I use 'folders' instead of 'groups' and so new files don't need to be added to the project file explicitly. Codex however wastes a lot of time trying to find and then add files to the project and at times will get confused when it cannot find a file referenced in the list. I need to frequently steer it away from this confusion by adding a line We're using Xcode 16's folders that automatically add new files to the app's target...

I'd like to be able to optionally add this or another instruction to the final plan.
